We are unable to accept requests for repeat medication over the telephone. Please use the counterfoil provided and mark where indicated. Please do not write your request on scraps of paper as this can cause confusion. If you lose your counterfoil, we will be happy to produce a duplicate for you.
Cricklade (Main Surgery)
To obtain repeat medication, please place the repeat counterfoil in the box at reception, or through the letterbox if the surgery is closed. Please remember that we require 3 working days notice to process prescriptions. Any prescription requested after 5pm will not be processed until the following day. You can collect your medication any time the surgery is open, although we request that where possible this is before 6.30pm. See the front of this booklet for opening times.
Please note that if you request a repeat prescription ealier than one week before your prescription is due this will not be processed until the due date. Should you need to order early please either put the reason on your repeat request or speak to our reception team who will be happy to help.
Ashton Keynes (Branch Surgery)
Please post your repeat prescription counterfoil in the box marked 'Surgery', which is situated on the right hand side of the main Village Hall door. We will collect your request at the time of the next surgery. Please allow 3 full working days for your prescription to be processed and your medication to be ordered and dispensed. We will deliver your medication for you to Ashton Keynes Surgery.
If you would prefer to collect your medication from Cricklade, please let us know at the time you place your request. If you are unsure of the day on which your medication is to be delivered, please telephone Cricklade and speak to the dispenser or a receptionist, who will be happy to help you.

Repeat Prescription Requests by E-mail
If you would like to take advantage of our e-mail service, please e-mail us at firstname.lastname@example.org. We will happily send you details of how to make your request. For all e-mailed requests we will need the name of the patient, date of birth, address and details of the medication required. A reply will be sent to acknowledge receipt of the request.
Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip. Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
