Friends and Family Test

The new friends and family test is available for completion online, as well as in the surgery. We would value your comments and any feedback to enable us to look at ways we may be able to improve our service. Please see the link under 'Have your say'

Online Registration

Did you know you could manage your prescriptions and book appointments online? All you need to do is complete a form at reception and show photographic ID.  If you lack confidence in logging in to your account please book an appointment to see our Medical Administrator who will be more than happy to sit and show you how this service works.

Improved Access - Evening and Weekend Appointments Available Now -

You can now access additional Primary Care Services in the evenings and at weekends. Improved Access is a Wiltshire-wide initiative where local practices are working together to offer increased availability of services and appointments in the evenings and the weekend. These appointments are available 18.30-20.00 Monday-Friday and Saturdays, Sundays and bank holidays. This collaboration by practices will ensure all patients in Wiltshire have this option, but this might mean you will be offered and appointment at another practice.

Appointment times will vary - some being face-to-face, some on the phone - and there will be a mixture of bookable and same-day appointments available across the week. We will be offering appointments with GPs as well as with other healthcare professionals, such as a Practice Nurse of Health Care Assistant. We are keen to balance the importance of continuity of care with convenient access for patients, while ensuring the service is safe. To book such an appointment, contact your local Surgery during normal opening times. A receptionist will explain the process and ensure the booking is appropriate. You will need to given consent to use this service.  For more information, please contact your usual GP surgery.

Named GP

You may be aware all practices are required to provide their patients with a named GP who will have overall responsibility for the care and support that the surgery provides to them. As one of our patients, Dr de Silva will be your named GP. This does not prevent you from seeing any other GP in the practice.

Your health record and sharing of information

Your health record includes your medical history, details about your medication and any allergies you may have. At a time of emergency or crisis it is important that clinicians can securely access your recent medical history so that they can understand the best way to help you. They have to give access to other NHS organisations. This means that if you need to see an out of hours doctor or require treatment at an acute hospital for example, your health records can be quickly and easily accessed by clinicians to help tailor the medical support you need. Your information will only be shared with other healthcare services if you give your explicit consent. You will only be asked to share your medical information if the clinician feels it will benefit your care. If you do not want a clinician from another healthcare service to be able to view your health record you can say so at the point at which the clinician asks you. If you wish to opt out please ask reception for an op-out form, once completed we will record your preference in our computer system.

Bereavement support

Updated list of bereavement support organisations including two organisations providing support specifically for people affected by the pandemic.

  • National Bereavement Partnership - offer a telephone information and support service 7am-10pm daily and an online chat service. They also offer 12 sessions of counselling for 50 mins, free of charge. They note on their website ‘Our volunteers have a deep understanding for grief, loss and the very real anxiety surrounding COVID-19. They have the background knowledge and skills to be able to offer an effective, informative and efficient service to callers.’, Tel: 0808 448 0800
