New Patient Registration
We are currently unable to accept registrations from patients who live outside our registration area, or retain patients who move outside our area. We may review this, but at the moment due to capacity we are only able to accept registrations from patients moving into our area. Dec. 14
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photographic ID.
As a guide, we cover the area in and between Cricklade, Minety, Ashton Keynes, Down Ampney, Marston Meysey and Castle Eaton. Please see the 'practice boundary' map for further details. The green shaded area indicates the extent of our boundary for patients who wish to register with the practice. Our reception staff will be able to help with any queries. A recent change has meant that patients registered with the practice,who move to live within the outer boundary (purple line), may, if they chose, remain registered with the practice. Again, please ask one of our receptionists if you have any queries.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
Practice Boundary Map
You may print off a registration form, fill it out and bring it with you to register but please be aware that patients must come into the surgery in person with photo ID and evidence of address. There also may be further forms to complete during registration
